Accomplishments:

• Successfully completed Safety in Process Design workshops in April 2011, this workshops explained the issues associated with the process design of a facility and in developing “inherently safer” designs. Extensive case studies were presented that discussed incidents in the process industry. The study shows how process design, management of change, and inadequate safeguards have all contributed to major accidents.
• Initiated to work on the R&P safety database project. The database is an effective tool for industry to share lessons learned from safety incidents at operating plants to avoid recurrences. The development phase of the database has already been completed.

• Maintenance of the R&P safety database project by collecting lesson learnt sharing from members and uploading to website.
• Develop stewardship processverification and validation (V&V) protocol for CoP SHE Management for Major Shutdown and Turnaround Activity implementation. The verification and validation is the process of checking that a product, service, or system meets specifications and that it fulfills its intended purpose. These are critical components of a quality management system.
• Develop “Winning Excellence Safety Culture” recommended practice and share among members.
